Ce sont leurs mots, leurs exp&#233;riences, leurs analyses critiques, et c'est pr&#233;cis&#233;ment pour cela qu'ils comptent.&lt;/p&gt;
&lt;p&gt;Le projet Princesse s'appuyait sur une clinique mobile de sant&#233; sexuelle qui se d&#233;pla&#231;ait directement sur les sites de prostitution de la r&#233;gion de San-Pedro, en C&#244;te d'Ivoire. Pour le mener &#224; bien, nous avons collabor&#233; avec l'ONG Aprosam et une &#233;quipe m&#234;lant m&#233;decins, biologistes, assistantes sociales&#8230; et paires &#233;ducatrices. Ces derni&#232;res ne sont pas de simples b&#233;n&#233;ficiaires&#160;: elles viennent de la communaut&#233; concern&#233;e et en sont des actrices essentielles. Elles connaissent les r&#233;alit&#233;s du terrain mieux que quiconque, et leurs retours compl&#232;tent ce que nous, chercheurs et soignants, ne percevons pas toujours.&lt;/p&gt;
&lt;p&gt;Leur r&#244;le est d&#233;terminant&#160;: elles recrutent les participantes, les sensibilisent &#224; des outils comme la PrEP orale, les mobilisent pour qu'elles se rendent au camion Princesse et assurent un suivi afin que leur parcours de soins s'inscrive dans la dur&#233;e. Elles nous expliquent aussi, lors d'ateliers comme celui-ci, les obstacles &#224; l'adh&#233;sion au projet ou &#224; la PrEP orale, ce qui est indispensable. En effet, en tant que scientifiques, nous sommes souvent convaincus de la pertinence des solutions propos&#233;es. Mais seule la r&#233;alit&#233; de terrain nous permet de r&#233;aliser que leur adoption n'a rien d'automatique. Le savoir exp&#233;rientiel des paires &#233;ducatrices est alors pr&#233;cieux et essentiels pour mieux comprendre les logiques &#224; l'&#339;uvre.&lt;/p&gt;
&lt;p&gt;C'est gr&#226;ce &#224; elles, par exemple, que nous avons compris pourquoi la prise de PrEP orale n'est pas si simple&#160;: ce n'est pas toujours la priorit&#233; des participantes, qui doivent d'abord, dans un contexte de forte pr&#233;carit&#233;, g&#233;rer d'autres priorit&#233;s quotidiennes. Les contraintes concr&#232;tes de l'offre de pr&#233;vention ne contrebalancent pas forc&#233;ment un b&#233;n&#233;fice plus &#171;&lt;small class=&#034;fine d-inline&#034;&gt;&#160;&lt;/small&gt;th&#233;oriques&lt;small class=&#034;fine d-inline&#034;&gt;&#160;&lt;/small&gt;&#187;.&lt;/p&gt;
&lt;p&gt;Les retours des paires-&#233;ducatrices nous ont permis d'ajuster l'offre de soins. Par exemple, elles nous ont alert&#233;s sur des aspects tr&#232;s concrets, comme la n&#233;cessit&#233; d'offrir une collation apr&#232;s les consultations comportant de nombreuses prises de sang. Nous pensions qu'un acc&#232;s gratuit aux examens et aux soins suffirait, mais certaines femmes avaient le sentiment d'&#234;tre flou&#233;es&#160;: lorsqu'elles donnent leur sang ailleurs, on leur offre toujours quelque chose &#224; boire ou &#224; manger. Un d&#233;tail simple, pourtant essentiel. D'autres ateliers ont enfin permis de repenser les horaires, de d&#233;placer certains sites d'intervention ou de r&#233;organiser des &#233;tapes du parcours de soins.&lt;/p&gt;
&lt;p&gt;Obtenir des retours sinc&#232;res n'a cependant pas &#233;t&#233; imm&#233;diat. Le contexte joue un r&#244;le important&#160;: les messages des bailleurs p&#232;sent fortement sur les programmes, et le rapport de domination entre savoir scientifique reconnu et savoir exp&#233;rientiel des paires &#233;ducatrices ne doit pas &#234;tre n&#233;glig&#233;. Il nous a alors fallu du temps pour instaurer une relation de confiance, un espace o&#249; les paires &#233;ducatrices se sentent l&#233;gitimes pour d&#233;passer les discours attendus et exprimer leur v&#233;ritable opinion. Nos ateliers en commun ont &#233;t&#233; peu productifs au d&#233;part, mais absolument n&#233;cessaires pour la suite.&lt;/p&gt;
&lt;p&gt;Et la suite, c'est un travail de coconstruction des interventions, o&#249; savoir scientifique et savoir exp&#233;rientiel se r&#233;pondent pour identifier les leviers et les freins &#224; l'adoption de nouveaux outils, et adapter les solutions au mieux. Les communaut&#233;s connaissent leur terrain mieux que nous, chercheurs, qui ne sommes pas sur place en permanence. &#201;couter les premi&#232;res concern&#233;es est donc indispensable. L'empouvoirement prend du temps, mais rien ne peut se faire sans les paires &#233;ducatrices. Leur parole est notre boussole.&lt;/p&gt;

&lt;p&gt;&lt;strong&gt;METHODS:&lt;/strong&gt; This analysis included 400 FSWs eligible for PrEP (HIV-negative, HBsAg-negative) enrolled up to December 2022 (&gt;6 months of follow-up). Clinical records were analyzed to describe PrEP cascade, follow-up, and PrEP trajectories. A multivariable logistic regression identified factors associated with PrEP early adopters.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;RESULTS:&lt;/strong&gt; Although 98% of eligible FSWs were interested in PrEP, only 62% initiated PrEP, and 39% renewed it at least once.&lt;br class='autobr' /&gt;
Follow-up was very short (&lt;6 months) for 48%, short (6-12 months) for 15%, seasonal (&gt;12 months with a gap of 6 months between two visits) for 31%, and regular (&gt;12 months with no gap) for 6%.&lt;br class='autobr' /&gt;
PrEP initiation was high among FSWs with regular (96%), seasonal (83%), or short (80%) follow-up, vs. 39% for those with very short follow-up. Among those who initiated, only 88% (regular), 70% (seasonal), 74% (short), and 35% (very short) renewed PrEP.&lt;br class='autobr' /&gt;
Among 148 FSWs with regular/seasonal follow-up, four PrEP trajectories were identified (cf. figure showing individual trajectories): 15% &#8220;never initiated&#8221; PrEP, 39% &#8220;initiated and were later not interested anymore&#8221;, 18% &#8220;re-initiated after non-interest&#8221;, and 28% &#8220;initiated and never expressed non-interest&#8221;, the last two groups being considered as &#8220;early adopters&#8221;. Factors associated with early PrEP adoption included the usual price of intercourse with clients &lt;1500 FCFA (aOR=2.4 [1.2-4.8]) and working in brothels (aOR=3.2 [1.6-6.6]).&lt;/p&gt;

&lt;p&gt;&lt;strong&gt;CONCLUSIONS:&lt;/strong&gt; In PRINCESSE, loss-to-follow-up and seasonality of risks limited PrEP adoption. FSWs who were more precarious and less mobile became early adopters of this new prevention tool and could serve as potential ambassadors for promoting its uptake.&lt;/p&gt;

&lt;p&gt;A total of 489 women were enrolled in the PRINCESSE cohort. This analysis included 400 FSWs eligible for PrEP (HIV-negative, HBsAg-negative at baseline) enrolled up to December 2022 (i.e. enrolled at least 6 months before cohort closure). Clinical records were analyzed to describe PrEP cascade, follow-up, and PrEP trajectories. A multivariable logistic regression identified factors associated with PrEP early adopters.&lt;/p&gt;

Perception de la Prophylaxie pr&#233;exposition &#224; longue dur&#233;e d'action chez les travailleuses du sexe et les HSH &#224; San Pedro et Abidjan, C&#244;te d'Ivoire&lt;/h2&gt;
&lt;p&gt;&lt;strong&gt;Introduction&#160;:&lt;/strong&gt; Des recherches ont montr&#233; que l'utilisation et l'adh&#233;sion &#224; la PrEP par voie orale peuvent &#234;tre complexes au sein de certaines populations, telles que les travailleuses du sexe (TS) et les hommes ayant des relations sexuelles avec d'autres hommes (HSH), notamment en raison de leur grande mobilit&#233;, d'une vie quotidienne impr&#233;visible ou d'effets secondaires. Dans ce contexte, la PrEP &#224; longue dur&#233;e d'action (actuellement &#224; diff&#233;rents stades de d&#233;veloppement), telle que les injectables, le gel, les implants, les suppositoires vaginaux, les douches vaginales/anales ou les anneaux vaginaux, pourraient contribuer &#224; surmonter certaines difficult&#233;s rencontr&#233;es avec la PrEP orale. Cette &#233;tude visait &#224; explorer les perceptions de ces formulations &#224; longue dur&#233;e d'action parmi les TS et les HSH en C&#244;te d'Ivoire.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;M&#233;thodologie&#160;:&lt;/strong&gt; Inscrite dans le projet ANRS 12381 PRINCESSE, cette sous-&#233;tude qualitative a &#233;t&#233; men&#233;e en novembre-d&#233;cembre 2023 au sein de deux ONG communautaires&#160;: Aprosam &#224; San Pedro et Espace Confiance &#224; Abidjan aupr&#232;s des TS et HSH. Au total, 32 TS (19 na&#239;ves de PrEP et 13 exp&#233;riment&#233;es) et 12 HSH (3 na&#239;fs de PrEP et 9 exp&#233;riment&#233;s) ont &#233;t&#233; interrog&#233;s (lors de14 entretiens individuels et 5 focus groups). Une analyse th&#233;matique s'est concentr&#233;e sur l'aspect pratique, la discr&#233;tion, l'autonomie, l'efficacit&#233; per&#231;ue et les effets secondaires anticip&#233;s des diff&#233;rentes formulations de PrEP &#224; longue dur&#233;e d'action.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;R&#233;sultats&#160;:&lt;/strong&gt; Les participants ont g&#233;n&#233;ralement exprim&#233; des opinions favorables &#224; l'&#233;gard de la PrEP &#224; longue dur&#233;e d'action, consid&#233;r&#233;e comme une forme moins contraingrante que la PrEP par voie orale. Les injections ont &#233;t&#233; particuli&#232;rement appr&#233;ci&#233;es pour leur discr&#233;tion (pas de marque visible), les injections semestrielles &#233;tant pr&#233;f&#233;r&#233;es. Quelques inqui&#233;tudes ont &#233;t&#233; exprim&#233;es quant &#224; la douleur li&#233;e &#224; l'injection. Le gel, assimil&#233; &#224; un lubrifiant, a s&#233;duit par sa double fonction de protection et de plaisir. Toutefois, certains participants ont exprim&#233; des doutes quant &#224; son efficacit&#233;. Bien que la dur&#233;e d'action de l'implant (1 an) ait &#233;t&#233; appr&#233;ci&#233;e, elle n'a pas &#233;t&#233; pl&#233;biscit&#233;e en raison du manque de discr&#233;tion (cicatrice) et des effets secondaires potentiels anticip&#233;s. L'autonomie et la facilit&#233; d'utilisation des suppositoires vaginaux sont per&#231;ues comme avantageuses, &#224; condition que les conditions d'hygi&#232;ne soient optimales&lt;small class=&#034;fine d-inline&#034;&gt;&#160;&lt;/small&gt;; les HSH font le parall&#232;le avec les suppositoires anaux. M&#234;me si les douches vaginales sont largement pratiqu&#233;es par les TS, la lotion de nettoyage intime de la PrEP a re&#231;u des r&#233;actions mitig&#233;es de la part des TS, conscientes des risques d'un usage excessif. Enfin, l'anneau vaginal a suscit&#233; peu d'int&#233;r&#234;t en raison de la m&#233;connaissance de l'outil.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Conclusion&#160;:&lt;/strong&gt; Dans l'ensemble, la PrEP &#224; longue dur&#233;e d'action peut offrir des options alternatives pour la pr&#233;vention du VIH. Les &#233;l&#233;ments cl&#233;s favorisant l'adoption &#224; un format (PrEP &#224; longue dur&#233;e d'action) sont la facilit&#233; d'utilisation, un suivi simplifi&#233; (le moins souvent possible), des r&#232;gles claires pour le d&#233;marrage et l'arr&#234;t, la discr&#233;tion, l'efficacit&#233; per&#231;ue et les effets secondaires anticip&#233;s.&lt;/p&gt;

Abstract&lt;/h2&gt;
&lt;p&gt;&lt;strong&gt;Background:&lt;/strong&gt; Female sex workers (FSWs) are at high risk of contracting STIs, in particular in Sub-Saharan Africa. The implementation of oral HIV pre-exposure prophylaxis provided an opportunity to draw attention to the sexual health needs of FSWs. Innovative strategies to screen for and reduce the burden of STIs is thus a priority. This study describes STI screening among FSWs enrolled in the PRINCESSE project in C&#244;te d'Ivoire.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Methods:&lt;/strong&gt; The PRINCESSE project (2019&#8211;2023) was an interventional cohort of FSWs &#8805;18&#8239;years, evaluating a comprehensive, community-based sexual and reproductive health care package, including the management of STIs, offered through mobile clinics operating on prostitution sites in San Pedro area. HIV testing and syndromic STI testing were offered at baseline and every 3&#8239;months. Biological testing of Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) was offered annually. Clinical forms included sociodemographic, behavioral and sex-work-related characteristics. We describe baseline characteristics, coverage of clinical examination, and vaginal, anal swab collection. Social, behavioral and sex work-related factors associated with an STI syndromic diagnosis were explored. A multivariable logistic regression model was used to identify factors associated with diagnosing a symptomatic STI.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Results:&lt;/strong&gt; 489 FSWs were included in the PRINCESSE cohort. Median age was 29&#8239;years (24&#8211;35&#8239;years), 28.6% had had sex without a condom in the last 7&#8239;days. The prevalence of HIV at baseline was 10.5%. Only one case of HIV seroconversion was observed during the project. The most frequent symptom was ano-vaginal discharge (19.1%). The prevalence of STI based on clinical symptoms was 26.6%. The proportion of vaginal swab samples for which the PCR result was positive was 8.0% for CT and 4.0% for NG. Only age remained significantly associated with diagnosing a symptomatic STI in the multivariable analysis.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Conclusion:&lt;/strong&gt; This study revealed a high prevalence of HIV and STIs, similar to national estimates among FSWs enrolled in a sexual health cohort. Screening for these generically asymptomatic bacterial STIs must be combined with the syndromic approach used in key populations, especially with the introduction of new PrEP programs, to reduce the exposure of individuals in these populations to STIs.&lt;/p&gt;&lt;/div&gt;

&lt;p&gt;Le projet Princesse a d&#233;velopp&#233;, document&#233; et analys&#233; une offre globale de sant&#233; sexuelle et reproductive incluant la pr&#233;vention combin&#233;e du VIH, aupr&#232;s d'une cohorte de travailleuses du sexe &#224; San Pedro en C&#244;te d'Ivoire. Cette offre de sant&#233;, mise en place entre novembre 2019 et juin 2023, impliquait un suivi m&#233;dical trimestriel, notamment en raison de la d&#233;livrance de la prophylaxie pr&#233;-exposition (PrEP), m&#233;dicament qui prot&#232;ge du VIH lors de rapports sexuels sans pr&#233;servatif. Toutefois, une grande partie des femmes incluses dans cette cohorte ont connu des interruptions dans leur suivi m&#233;dical et donc dans leur traitement, &#224; plus ou moins long terme. D'une part, le suivi a &#233;t&#233; compliqu&#233; par le contexte de la r&#233;gion de San Pedro&#160;: r&#233;put&#233;e pour la culture du caf&#233; et du cacao, elle attire ainsi entre septembre et d&#233;cembre de nombreux travailleurs saisonniers dans les champs, mais &#233;galement des travailleuses du sexe. Une partie des femmes incluses dans cette p&#233;riode de l'ann&#233;e repartaient, &#224; la fin de la saison de r&#233;colte, dans leur r&#233;gion d'origine et nombreuses &#233;taient les perdues de vue, jusqu'&#224; l'ann&#233;e suivante ou d&#233;finitivement. D'autre part, dans les p&#233;riodes de l'ann&#233;e o&#249; l'activit&#233; diminue, de nombreuses femmes r&#233;sidant &#224; San Pedro voyagent plusieurs semaines pour visiter leur famille ou un client r&#233;gulier, dans d'autres r&#233;gions ivoiriennes ou dans leur pays d'origine (au Nigeria souvent, mais &#233;galement dans des pays francophones de la sous-r&#233;gion), interrompant alors leur prise de PrEP ou manquant leur rendez-vous trimestriel, ce qui complique le suivi m&#233;dical (d&#233;pistage et traitement des infections sexuellement transmissibles dont le VIH notamment).&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Objectif&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;L'objectif de cette communication est d'&#233;tudier les mobilit&#233;s temporaires ou saisonni&#232;res des femmes incluses dans la cohorte Princesse, et les cons&#233;quences qu'elles ont sur l'observance de la PrEP, et plus largement sur leur suivi m&#233;dical.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Donn&#233;es et m&#233;thodes&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;L'ensemble de soins de sant&#233; sexuelle et reproductive du projet Princesse a &#233;t&#233; propos&#233; dans une clinique communautaire et via une clinique mobile op&#233;rant sur 10 sites de prostitution (visit&#233;s toutes les deux semaines) pendant 45 mois (d&#233;cembre 2019 &#8211; juin 2023). L'analyse repose sur des entretiens qualitatifs r&#233;alis&#233;s avec 38 travailleuses du sexe en d&#233;cembre 2019, septembre 2021, mars et mai 2022, ainsi que sur les questionnaires socio-comportementaux compl&#233;t&#233;s par les femmes tous les six mois au cours de leur suivi. Les trajectoires de mobilit&#233;, d&#233;crites pr&#233;cis&#233;ment dans ces questionnaires, seront mises en regard avec les trajectoires de PrEP (initiation, observance, interruption ou arr&#234;t d&#233;finitif) et les trajectoires de suivi m&#233;dical (r&#233;gularit&#233; du suivi, perte de vue), retrac&#233;es gr&#226;ce aux fiches cliniques remplies par l'&#233;quipe m&#233;dicale.&lt;/p&gt;&lt;/div&gt;

&lt;p&gt;&lt;strong&gt;Objectives&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;To estimate the epidemiological impact of past HIV interventions and the magnitude and contribution of undiagnosed HIV among different risk groups on new HIV acquisitions in C&#244;te d'Ivoire, Mali and Senegal.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Design&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;HIV transmission dynamic models among the overall population and key populations [female sex workers (FSW), their clients, and MSM].&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Methods&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;Models were independently parameterized and calibrated for each set of country-specific demographic, behavioural, and epidemiological data. We estimated the fraction of new HIV infections over 2012&#8211;2021 averted by condom use and antiretroviral therapy (ART) uptake among key population and nonkey population, the direct and indirect contribution of specific groups to new infections [transmission population-attributable fraction (tPAF)] over 2012&#8211;2021 due to prevention gaps, and the distribution of undiagnosed PWH by risk group in January 2022 and their tPAF over 2022&#8211;2031.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Results&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;Condom use and ART may have averted 81&#8211;88% of new HIV infections over 2012&#8211;2021 across countries, mostly because of condom use by key population. The tPAF of all key populations combined over 2012&#8211;2021 varied between 27% (C&#244;te d'Ivoire) and 79% (Senegal). Male key population (clients of FSW and MSM) contributed most to new infections (&gt;60% in Mali and Senegal) owing to their higher HIV prevalence and larger prevention gaps. In 2022, men represented 56% of all PWH with an undiagnosed infection in C&#244;te d'Ivoire (male key population = 15%), 46% in Mali (male key population = 23%), and 69% in Senegal (male key population = 55%). If HIV testing and ART initiation rates remain at current levels, 20% of new HIV infections could be due to undiagnosed key population PWH in C&#244;te d'Ivoire over 2022&#8211;2031, 53% in Mali, and 65% in Senegal.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Conclusion&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;Substantial HIV diagnosis gaps remain in Western Africa, especially among male key population. Addressing these gaps is key to impacting the HIV epidemics in the region and achieving the goal of ending AIDS by 2030.&lt;/p&gt;&lt;/div&gt;

Abstract&lt;/h2&gt;
&lt;p&gt;&lt;strong&gt;Background&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;Coverage of HIV testing remains sub-optimal in West Africa. Between 2019 and 2022, the ATLAS program distributed&#160;400 000 oral HIV self-tests (HIVST) in C&#244;te d'Ivoire, Mali, and Senegal, prioritising female sex workers (FSW) and men having sex with men (MSM), and relying on secondary redistribution of HIVST to partners, peers and clients to reach individuals not tested through conventional testing. This study assesses the proportion of first-time testers among HIVST users and the associated factors.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Methods&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;A phone-based survey was implemented among HIVST users recruited using dedicated leaflets inviting them to anonymously call a free phone number. We collected socio-demographics, sexual behaviours, HIV testing history, HIVST use, and satisfaction with HIVST. We reported the proportion of first-time testers and computed associated factors using logistic regression.&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Results&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;Between March and June 2021, 2 615 participants were recruited for 50 940 distributed HIVST (participation rate: 5.1%). Among participants, 30% received their HIVST kit through secondary distribution (from a friend, sexual partner, family member, or colleague).&lt;/p&gt;
&lt;p&gt;The proportion who had never tested for HIV before HIVST (first-time testers) was 41%. The main factors associated with being a first-time tester were sex, age group, education level, condom use, and secondary distribution. A higher proportion was observed among those aged 24 years or less (55% vs 32% for 25&#8211;34, aOR: 0.37 [95%CI: 0.30&#8211;0.44], and 26% for 35 years or more, aOR: 0.28 [0.21&#8211;0.37]); those less educated (48% for none/primary education vs 45% for secondary education, aOR: 0.60 [0.47&#8211;0.77], and 29% for higher education, aOR: 0.33 [0.25&#8211;0.44]). A lower proportion was observed among women (37% vs 43%, aOR: 0.49 [0.40&#8211;0.60]); those reporting always using a condom over the last year (36% vs 51% for those reporting never using them, aOR: 2.02 [1.59&#8211;2.56]); and those who received their HISVST kit through primary distribution (39% vs 46% for secondary distribution, aOR: 1.32 [1.08&#8211;1.60]).&lt;/p&gt;
&lt;p&gt;&lt;strong&gt;Conclusion&lt;/strong&gt;&lt;/p&gt;
&lt;p&gt;ATLAS HIVST strategy, including secondary distribution, successfully reached a significant proportion of first-time testers. HIVST has the potential to reach underserved populations and contribute to the expansion of HIV testing services in West Africa.&lt;/p&gt;&lt;/div&gt;
