Joseph Larmarange échange avec les paires éducatrices du projet PRINCESSE en Côte d'Ivoire sur les difficultés de terrain dans la mise en œuvre.

Ici, je discute avec une partie des douze paires éducatrices du projet Princesse, lors d'un atelier de réflexion collective. Hors champ se trouve le reste de l'équipe scientifique qui participe également à cet échange. Et derrière nous, un mur couvert de post-it : bleu pour ce qui fonctionne, rose pour ce qui coince, jaune pour ce qu'il faut améliorer. Ce sont leurs mots, leurs expériences, leurs analyses critiques, et c'est précisément pour cela qu'ils comptent.

Le projet Princesse s'appuyait sur une clinique mobile de santé sexuelle qui se déplaçait directement sur les sites de prostitution de la région de San-Pedro, en Côte d'Ivoire. Pour le mener à bien, nous avons collaboré avec l'ONG Aprosam et une équipe mêlant médecins, biologistes, assistantes sociales… et paires éducatrices. Ces dernières ne sont pas de simples bénéficiaires : elles viennent de la communauté concernée et en sont des actrices essentielles. Elles connaissent les réalités du terrain mieux que quiconque, et leurs retours complètent ce que nous, chercheurs et soignants, ne percevons pas toujours.

Leur rôle est déterminant : elles recrutent les participantes, les sensibilisent à des outils comme la PrEP orale, les mobilisent pour qu'elles se rendent au camion Princesse et assurent un suivi afin que leur parcours de soins s'inscrive dans la durée. Elles nous expliquent aussi, lors d'ateliers comme celui-ci, les obstacles à l'adhésion au projet ou à la PrEP orale, ce qui est indispensable. En effet, en tant que scientifiques, nous sommes souvent convaincus de la pertinence des solutions proposées. Mais seule la réalité de terrain nous permet de réaliser que leur adoption n'a rien d'automatique. Le savoir expérientiel des paires éducatrices est alors précieux et essentiels pour mieux comprendre les logiques à l'œuvre.

C'est grâce à elles, par exemple, que nous avons compris pourquoi la prise de PrEP orale n'est pas si simple : ce n'est pas toujours la priorité des participantes, qui doivent d'abord, dans un contexte de forte précarité, gérer d'autres priorités quotidiennes. Les contraintes concrètes de l'offre de prévention ne contrebalancent pas forcément un bénéfice plus « théoriques ».

Les retours des paires-éducatrices nous ont permis d'ajuster l'offre de soins. Par exemple, elles nous ont alertés sur des aspects très concrets, comme la nécessité d'offrir une collation après les consultations comportant de nombreuses prises de sang. Nous pensions qu'un accès gratuit aux examens et aux soins suffirait, mais certaines femmes avaient le sentiment d'être flouées : lorsqu'elles donnent leur sang ailleurs, on leur offre toujours quelque chose à boire ou à manger. Un détail simple, pourtant essentiel. D'autres ateliers ont enfin permis de repenser les horaires, de déplacer certains sites d'intervention ou de réorganiser des étapes du parcours de soins.

Obtenir des retours sincères n'a cependant pas été immédiat. Le contexte joue un rôle important : les messages des bailleurs pèsent fortement sur les programmes, et le rapport de domination entre savoir scientifique reconnu et savoir expérientiel des paires éducatrices ne doit pas être négligé. Il nous a alors fallu du temps pour instaurer une relation de confiance, un espace où les paires éducatrices se sentent légitimes pour dépasser les discours attendus et exprimer leur véritable opinion. Nos ateliers en commun ont été peu productifs au départ, mais absolument nécessaires pour la suite.

Et la suite, c'est un travail de coconstruction des interventions, où savoir scientifique et savoir expérientiel se répondent pour identifier les leviers et les freins à l'adoption de nouveaux outils, et adapter les solutions au mieux. Les communautés connaissent leur terrain mieux que nous, chercheurs, qui ne sommes pas sur place en permanence. Écouter les premières concernées est donc indispensable. L'empouvoirement prend du temps, mais rien ne peut se faire sans les paires éducatrices. Leur parole est notre boussole.

Abstract

BACKGROUND: The ANRS 12381 PRINCESSE project (11/2019-06/2023) enrolled 489 FSWs and implemented community-based sexual and reproductive health care, including PrEP, delivered through mobile clinics at prostitution sites in the San Pedro region. This presentation aims to describe PrEP trajectories and the factors associated with its use, focusing on “early adopters”.

METHODS: This analysis included 400 FSWs eligible for PrEP (HIV-negative, HBsAg-negative) enrolled up to December 2022 (>6 months of follow-up). Clinical records were analyzed to describe PrEP cascade, follow-up, and PrEP trajectories. A multivariable logistic regression identified factors associated with PrEP early adopters.

RESULTS: Although 98% of eligible FSWs were interested in PrEP, only 62% initiated PrEP, and 39% renewed it at least once.
Follow-up was very short (<6 months) for 48%, short (6-12 months) for 15%, seasonal (>12 months with a gap of 6 months between two visits) for 31%, and regular (>12 months with no gap) for 6%.
PrEP initiation was high among FSWs with regular (96%), seasonal (83%), or short (80%) follow-up, vs. 39% for those with very short follow-up. Among those who initiated, only 88% (regular), 70% (seasonal), 74% (short), and 35% (very short) renewed PrEP.
Among 148 FSWs with regular/seasonal follow-up, four PrEP trajectories were identified (cf. figure showing individual trajectories): 15% “never initiated” PrEP, 39% “initiated and were later not interested anymore”, 18% “re-initiated after non-interest”, and 28% “initiated and never expressed non-interest”, the last two groups being considered as “early adopters”. Factors associated with early PrEP adoption included the usual price of intercourse with clients <1500 FCFA (aOR=2.4 [1.2-4.8]) and working in brothels (aOR=3.2 [1.6-6.6]).

CONCLUSIONS: In PRINCESSE, loss-to-follow-up and seasonality of risks limited PrEP adoption. FSWs who were more precarious and less mobile became early adopters of this new prevention tool and could serve as potential ambassadors for promoting its uptake.

Abstract

Background: Regular testing is recognized as a key strategy for HIV control. The 2023 Integrated Biological and Behavioral Survey (IBBS) in Cambodia revealed that nearly one-third of men who have sex with men (MSM) and one-fourth of transgender women (TGW) were never tested for HIV or not for more than 12 months. The majority of MSM and TGW were tested at community-based organizations (CBOs) facilities and by CBO outreach workers, while private facilities are poorly used for HIV testing (6% for MSM and 9% for TGW). Private pharmacies could be able to deliver HIV self-testing kits giving the advantage of confidentiality, anonymity, and time savings, in particular for those reluctant to visit CBOs. The recruitment of participants using a respondent-driven sampling method could provide the opportunity to reach MSM and TGW outside the network of CBOs.

Objective: This pilot study aims to evaluate the feasibility of free HIV self-testing delivery by a private pharmacy combined with a respondent-driven sampling method to improve HIV testing among MSM and TGW in Phnom Penh, Cambodia.

Methods: Both qualitative and quantitative approaches are used in this prospective feasibility study. The protocol was approved by the National Ethics Committee for Health Research in Cambodia (N0 351 NECHR). MSM and TGW aged more than 18 years old will be recruited via a respondent-driven sampling method with seeds recruited at hot spots and on social networks. The seeds will then distribute electronic and paper coupons to their networks physically and via social media, messaging, and calling applications. Each recruited peer will bring the coupon to receive direct and free access to one HIV self-testing kit at partner pharmacies as well as 10 additional coupons to recruit members of their networks. As for quantitative analysis, data from the study website will be imported, appended into a single matrix using Stata version 18SE (StataCorp), and analyzed using descriptive statistics, with a statistical significance level of .05. After 6 months, a qualitative assessment will be conducted among users, providers, and policymakers or key stakeholders to evaluate the acceptability and appropriateness of the strategy and to identify the barriers, facilitators, and recommendations. All transcripts will be analyzed according to the 6-phase reflexive thematic approach by Braun and Clarke. The results will be reported by participant-based and provider-based issues. QSR NVivo V.14 for Windows will be used to manage the data.

Results: The study was funded by Agence nationale de recherches sur le sida, les hépatites, et les maladies infectieuses émergentes (ANRS) in September 2023. Approval of the study protocol was successfully obtained from the NECHR in Cambodia and the Commission Nationale de l'Informatique et des Libertés (CNIL) in France (Autorisation Tacite) in February 2025. Data collection will be conducted between September 2024 and December 2025. The initial results are expected to be published in February 2026.

Conclusions: This public-private partnership intervention could allow the hidden population of MSM and TGW in Phnom Penh to be reached and tested.

Primary objective

Evaluate the feasibility of HIV self-testing (HIVST) delivery by a private pharmacy network among men who have sex with men (MSM) and transgender women (TGW) recruited through a classic and digital Respondent Driven Sampling method to improve HIV testing in Phnom Penh, Cambodia

Secondary objectives

Evaluate the acceptability and appropriateness of the strategy
Identify barriers and facilitators
Estimate the linkage to confirmatory testing for those with a reactive test and linkage to HIV care and ART for those with a positive confirmatory HIV test
Estimate linkage to pre-exposure prophylaxis (PrEP) services for negative participants
Estimate the characteristics of participants and compare to those reported in Integrated Biological and Behavioral Survey (IBBS 2019) for MSM and TGW in Cambodia
Evaluate the adherence of participants to a 6-monthly repeated HIV testing

Methodology

An interventional pilot study using a mixed qualitative and quantitative approach will be carried out in order to evaluate the feasibility, acceptability and appropriateness of the strategy and to identify barriers and facilitators.

The participant recruitment method will be designed as a Respondent Driven Sampling (RDS) by recruiting initial seeds both at hotspots and on social networks. Prior to seed recruitment, a consultative meeting will be conducted in order to optimise the selection criteria of the seeds. The seeds will then distribute electronic and paper coupons to their networks physically and via social media, messaging and calling applications such as Twitter, Whatapp and Telegram (e-coupons). Each recruited participant will bring the coupon to be scanned at partner pharmacies to receive direct and free access to one HIVST kit. Partner pharmacies with a proper dedicated room/space securing privacy and secrecy in Phnom Penh are the study sites. There, trained community workers who get notified by partner at each visit of participant will do the study visit including information/consent, e-CRF filling and give a link of an external website to send the HIV test results and receive information or advices. These recruited individuals will be considered as wave 1 of recruitment and will each receive 10 additional coupons to recruit the members of their networks. The next round of individuals recruited and enrolled will be considered as wave 2, and so on.

After 6 months, a qualitative assessment via focus groups will be conducted among MSM, TGW and pharmacists to evaluate feasibility, acceptability and appropriateness of the intervention and to identify barriers and facilitators.

Several ART sites/HIV clinics particularly in charge of key populations will be specifically selected as participating centers in order to collect information on linkage to care or prevention services for positive and negative participants, respectively.

Each participant will be followed during 18 months and encouraged to perform a 6-monthly HIV testing during this period.

Expected enrolment

A pragmatic approach is carried out in which the enrolment will be stopped at the end of the inclusion period or if the number of participants reaches a maximum of 1500.

Abstract

Background

Pre-exposure prophylaxis (PrEP) is recommended by the WHO for HIV prevention among female sex workers (FSWs). A study conducted in 2016–2017 in Côte d'Ivoire showed that if PrEP is acceptable, FSWs also have many uncovered sexual health needs. Based on this evidence, the ANRS 12381 PRINCESSE project was developed in collaboration with a community-based organization. The main objective is to develop, document, and analyze a comprehensive sexual and reproductive healthcare package among FSWs in Côte d'Ivoire.

Methods

PRINCESSE is an open, single-arm interventional cohort of 500 FSWs in San Pedro (Côte d'Ivoire) and its surroundings. Recruitment started on November 26^th, 2019 and is ongoing; the cohort is planned to last at least 30 months. The healthcare package (including HIV, hepatitis B, and sexually transmitted infection management, pregnancy screening, and contraception) is available both at mobile clinics organized for a quarterly follow-up (10 intervention sites, each site being visited every two weeks) and at a fixed clinic.

Four waves of data collection were implemented: (i) clinical and safety data; (ii) socio-behavioral questionnaires; (iii) biological data; and (iv) in-depth interviews with female participants. Four additional waves of data collection are scheduled outside the cohort itself: (i) the medical and activity records of Aprosam for the PRINCESSE participants; (ii) the medical records of HIV+ FSW patients not participating in the PRINCESSE cohort, and routinely examined by Aprosam; (iii) in-depth interviews with key informants in the FSW community; and (iv) in-depth interviews with PRINCESSE follow-up actors.

Discussion

The PRINCESSE project is one of the first interventions offering HIV oral PrEP as part of a more global sexual healthcare package targeting both HIV- and HIV+ women. Second, STIs and viral hepatitis B care were offered to all participants, regardless of their willingness to use PrEP. Another innovation is the implementation of mobile clinics for chronic/quarterly care. In terms of research, PRINCESSE is a comprehensive, interdisciplinary project combining clinical, biological, epidemiological, and social specific objectives and outcomes to document the operational challenges of a multidisease program in real-life conditions.

Trial registration

The PRINCESSE project was registered on the Clinicaltrial.gov website (NCT03985085) on June 13, 2019.
