Abstract

Background: Regular testing is recognized as a key strategy for HIV control. The 2023 Integrated Biological and Behavioral Survey (IBBS) in Cambodia revealed that nearly one-third of men who have sex with men (MSM) and one-fourth of transgender women (TGW) were never tested for HIV or not for more than 12 months. The majority of MSM and TGW were tested at community-based organizations (CBOs) facilities and by CBO outreach workers, while private facilities are poorly used for HIV testing (6% for MSM and 9% for TGW). Private pharmacies could be able to deliver HIV self-testing kits giving the advantage of confidentiality, anonymity, and time savings, in particular for those reluctant to visit CBOs. The recruitment of participants using a respondent-driven sampling method could provide the opportunity to reach MSM and TGW outside the network of CBOs.

Objective: This pilot study aims to evaluate the feasibility of free HIV self-testing delivery by a private pharmacy combined with a respondent-driven sampling method to improve HIV testing among MSM and TGW in Phnom Penh, Cambodia.

Methods: Both qualitative and quantitative approaches are used in this prospective feasibility study. The protocol was approved by the National Ethics Committee for Health Research in Cambodia (N0 351 NECHR). MSM and TGW aged more than 18 years old will be recruited via a respondent-driven sampling method with seeds recruited at hot spots and on social networks. The seeds will then distribute electronic and paper coupons to their networks physically and via social media, messaging, and calling applications. Each recruited peer will bring the coupon to receive direct and free access to one HIV self-testing kit at partner pharmacies as well as 10 additional coupons to recruit members of their networks. As for quantitative analysis, data from the study website will be imported, appended into a single matrix using Stata version 18SE (StataCorp), and analyzed using descriptive statistics, with a statistical significance level of .05. After 6 months, a qualitative assessment will be conducted among users, providers, and policymakers or key stakeholders to evaluate the acceptability and appropriateness of the strategy and to identify the barriers, facilitators, and recommendations. All transcripts will be analyzed according to the 6-phase reflexive thematic approach by Braun and Clarke. The results will be reported by participant-based and provider-based issues. QSR NVivo V.14 for Windows will be used to manage the data.

Results: The study was funded by Agence nationale de recherches sur le sida, les hépatites, et les maladies infectieuses émergentes (ANRS) in September 2023. Approval of the study protocol was successfully obtained from the NECHR in Cambodia and the Commission Nationale de l'Informatique et des Libertés (CNIL) in France (Autorisation Tacite) in February 2025. Data collection will be conducted between September 2024 and December 2025. The initial results are expected to be published in February 2026.

Conclusions: This public-private partnership intervention could allow the hidden population of MSM and TGW in Phnom Penh to be reached and tested.

Abstract

Introduction

HIV epidemics in Western and Central Africa (WCA) remain concentrated among key populations, who are often unaware of their status. HIV self-testing (HIVST) and its secondary distribution among key populations, and their partners and relatives, could reduce gaps in diagnosis coverage.

We aimed to document and understand secondary HIVST distribution practices by men who have sex with men (MSM), female sex workers (FSW), people who use drugs (PWUD); and the use of HIVST by their networks in Côte d'Ivoire, Mali, and Senegal.

Methods

A qualitative study was conducted in 2021 involving (a) face-to-face interviews with MSM, FSW, and PWUD who received HIVST kits from peer educators (primary users) and (b) telephone interviews with people who received kits from primary contacts (secondary users). These individual interviews were audio-recorded, transcribed, and coded using Dedoose software. Thematic analysis was performed.

Results

A total of 89 participants, including 65 primary users and 24 secondary users were interviewed. Results showed that HIVST were effectively redistributed through peers and key populations networks. The main reported motivations for HIVST distribution included allowing others to access testing and protecting oneself by verifying the status of partners/clients. The main barrier to distribution was the fear of sexual partners' reactions. Findings suggest that members of key populations raised awareness of HIVST and referred those in need of HIVST to peer educators. One FSW reported physical abuse.

Secondary users generally completed HIVST within two days of receiving the kit. The test was used half the times in the physical presence of another person, partly for psychological support need. Users who reported a reactive test sought confirmatory testing and were linked to care. Some participants mentioned difficulties in collecting the biological sample (2 participants) and interpreting the result (4 participants).

Conclusion

The redistribution of HIVST was common among key populations, with minor negative attitudes. Users encountered few difficulties using the kits. Reactive test cases were generally confirmed. These secondary distribution practices support the deployment of HIVST to key populations, their partners, and other relatives. In similar WCA countries, members of key populations can assist in the distribution of HIVST, contributing to closing HIV diagnosis gaps.

Primary objective

Evaluate the feasibility of HIV self-testing (HIVST) delivery by a private pharmacy network among men who have sex with men (MSM) and transgender women (TGW) recruited through a classic and digital Respondent Driven Sampling method to improve HIV testing in Phnom Penh, Cambodia

Secondary objectives

Evaluate the acceptability and appropriateness of the strategy
Identify barriers and facilitators
Estimate the linkage to confirmatory testing for those with a reactive test and linkage to HIV care and ART for those with a positive confirmatory HIV test
Estimate linkage to pre-exposure prophylaxis (PrEP) services for negative participants
Estimate the characteristics of participants and compare to those reported in Integrated Biological and Behavioral Survey (IBBS 2019) for MSM and TGW in Cambodia
Evaluate the adherence of participants to a 6-monthly repeated HIV testing

Methodology

An interventional pilot study using a mixed qualitative and quantitative approach will be carried out in order to evaluate the feasibility, acceptability and appropriateness of the strategy and to identify barriers and facilitators.

The participant recruitment method will be designed as a Respondent Driven Sampling (RDS) by recruiting initial seeds both at hotspots and on social networks. Prior to seed recruitment, a consultative meeting will be conducted in order to optimise the selection criteria of the seeds. The seeds will then distribute electronic and paper coupons to their networks physically and via social media, messaging and calling applications such as Twitter, Whatapp and Telegram (e-coupons). Each recruited participant will bring the coupon to be scanned at partner pharmacies to receive direct and free access to one HIVST kit. Partner pharmacies with a proper dedicated room/space securing privacy and secrecy in Phnom Penh are the study sites. There, trained community workers who get notified by partner at each visit of participant will do the study visit including information/consent, e-CRF filling and give a link of an external website to send the HIV test results and receive information or advices. These recruited individuals will be considered as wave 1 of recruitment and will each receive 10 additional coupons to recruit the members of their networks. The next round of individuals recruited and enrolled will be considered as wave 2, and so on.

After 6 months, a qualitative assessment via focus groups will be conducted among MSM, TGW and pharmacists to evaluate feasibility, acceptability and appropriateness of the intervention and to identify barriers and facilitators.

Several ART sites/HIV clinics particularly in charge of key populations will be specifically selected as participating centers in order to collect information on linkage to care or prevention services for positive and negative participants, respectively.

Each participant will be followed during 18 months and encouraged to perform a 6-monthly HIV testing during this period.

Expected enrolment

A pragmatic approach is carried out in which the enrolment will be stopped at the end of the inclusion period or if the number of participants reaches a maximum of 1500.

Abstract

Background

Many respondent-driven sampling (RDS) methodologies have been employed to investigate hard-to-reach populations; however, these methodologies present some limits. We describe a minimally investigated RDS methodology in which peer recruitment and interviewing are phone-based. The feasibility of the methodology, field experiences, validity of RDS assumptions and characteristics of the sample obtained are discussed.

Methods

We conducted a phone-based RDS survey among men who have sex with men (MSM) aged 18 or above and living in Côte d'Ivoire. Eight initial MSM across Côte d'Ivoire were selected. Participants were asked to call a hotline to be registered and interviewed by phone. After the participants completed the questionnaire, they were asked to recruit a maximum of 3 MSM from their acquaintances.

Results

During the 9 months of the survey, 576 individuals called the hotline, and 518 MSM completed the questionnaire. The median delay between the invitation to participate and the completion of the questionnaire by peer-recruited MSM was 4 days [IQR: 1–12]. The recruitment process was not constant, with high variation in the number of people who called the hotline during the survey period.

RDS chain convergence to equilibrium was reached within 6 waves for most of the selected variables. For the network size estimation assumption, participants who incorrectly estimated their network size were observed.

Regarding the sample obtained, MSM were recruited from all the regions of Côte d'Ivoire with frequent interregional recruitment; 23.5% of MSM were recruited by someone who does not live in the same region. Compared to the MSM who participated in two other surveys in Côte d'Ivoire, the MSM in our sample were less likely to know about an MSM non-governmental organisation. However, MSM aged 30 years old and above and those with a low level of education were underrepresented in our sample.

Conclusion

We show that phone-based RDS surveys among MSM are feasible in the context of sub-Saharan Africa. Compared to other classical RDS survey methodologies, the phone-based RDS methodology seems to reduce selection bias based on geography and proximity with the MSM community. However, similar to other methodologies, phone-based RDS fails to reach older and less-educated MSM.

Contexte

Y compris en Afrique sub-saharienne, les hommes ayant des rapports sexuels avec d‘autres hommes (HSH) sont plus à risque de s'infecter par le VIH que la population générale. Dans ce groupe, les bisexuels sont souvent perçus comme plus « à risque » que les homosexuels exclusifs et pouvant constituer une « passerelle épidémique » avec la population générale, sur la base de l'hypothèse selon laquelle les bisexuels seraient moins susceptibles de se conformer aux normes préventives que les homosexuels exclusifs.

Objectif

Proposer une synthèse des connaissances sur les différentes catégories d'homo-bisexuels dans les enquêtes épidémiologiques réalisées en Afrique sub-saharienne depuis 2004 et sur leur exposition au VIH.

Méthodes

À partir d'une revue de la littérature, nous avons retenu les publications présentant des estimations quantifiées de la bisexualité, mesurée en fonction des enquêtes selon trois dimensions : le pôle d'activité sexuelle (sexe des partenaires sexuels sur une période donnée), l'identité déclarée et l'attirance sexuelle. Nous avons examiné : (i) les taux de bisexualité calculés selon ces trois dimensions ; (ii) les taux de prévalence du VIH par groupe (et les odds-ratio correspondants) et (iii) des indicateurs de comportements selon l'orientation sexuelle.

Résultats

Les taux de bisexualité varient fortement d'une étude à l'autre (de 12 à 94%) et selon l'indicateur retenu. Globalement, les taux observés en Afrique de l'Ouest sont sensiblement plus élevés que ceux d'Afrique australe, l'Afrique centrale et de l'Est étant dans une situation intermédiaire.

À l'exception de deux études, la prévalence du VIH observée parmi les bisexuels est inférieure à celle des homosexuels exclusifs, quelle que soit la dimension considérée pour définir ces deux groupes (voir figure). Les différences de prévalence selon l'identité déclarée sont plus marquées que selon le pôle d'activité sexuelle.

La majorité des études n'ont pas relevé de différence d'utilisation du préservatif entre bisexuels et homosexuels exclusifs. Les bisexuels ont parfois une fréquence de rapports sexuels moindre et systématiquement moins de rapports anaux réceptifs que les homosexuels exclusifs. Ils ont commencé leur vie sexuelle avec les hommes plus tardivement et ont eu moins de partenaires masculins au cours de leur vie. Par contre, ils sont moins nombreux à avoir déjà fait un test de dépistage du VIH.

Conclusion et recommandations

La bisexualité est multiforme en Afrique sub-saharienne : les bisexuels ne constituent pas un groupe homogène et les identités sexuelles sont si diverses qu'une simple opposition homosexuels vs. bisexuels est réductrice. Toutefois, selon les enquêtes disponibles et quelle que soit la dimension considérée pour mesurer la bisexualité, les bisexuels sont moins exposés au VIH que les homosexuels exclusifs. S'il reste important que les programmes de prévention et de traitement prennent en compte les bisexuels, les homosexuels exclusifs doivent rester une cible prioritaire pour l'accès aux nouveaux outils de la prévention et du soin.

Bisexualité masculine en Afrique subsaharienne : quels indicateurs dans les enquêtes quantitatives ?

Résumé

Les enquêtes quantitatives menées sur le continent africain depuis 2004 auprès des hommes ayant des rapports sexuels avec d'autres hommes (HSH) utilisent en majorité des approches du type « boules de neige » dont l'approche dite RDS (respondent driven survey). Dès lors, les individus les plus insérés dans un « milieu HSH » ont une plus grande probabilité d'être inclus, induisant une surreprésentation des jeunes et des homosexuels exclusifs.

La bisexualité y est mesurée soit au travers du sexe des partenaires ou pôle d'activité sexuel (calculé selon les articles sur les six derniers mois, la dernière année ou toute la vie), soit en fonction de l'identité déclarée des enquêtés (homosexuel/gay, bisexuel ou hétérosexuel, peu d'enquêtes proposant d'autres termes identifiants en langue locale). Plus rares, quelques enquêtes ont également mesuré l'attirance sexuelle (pour les hommes et/ou les femmes).

La part des bisexuels parmi les HSH africains est généralement beaucoup plus élevée qu'en Occident. Par rapport aux homosexuels exclusifs, les bisexuels ont des biographies sentimentales et sexuelles sensiblement différentes. Cependant, il ne s'agit pas d'une catégorie homogène que ce soit en termes de désirs, d'identité ou de projets de vie. Plusieurs auteurs ont souligné les limites d'une catégorisation binaire (homosexuels/bisexuels) ouvrant le débat sur quelles catégorisations alternatives utiliser à l'avenir dans les enquêtes quantitatives.
