Abstract

BACKGROUND: The ANRS 12381 PRINCESSE project (11/2019-06/2023) enrolled 489 FSWs and implemented community-based sexual and reproductive health care, including PrEP, delivered through mobile clinics at prostitution sites in the San Pedro region. This presentation aims to describe PrEP trajectories and the factors associated with its use, focusing on “early adopters”.

METHODS: This analysis included 400 FSWs eligible for PrEP (HIV-negative, HBsAg-negative) enrolled up to December 2022 (>6 months of follow-up). Clinical records were analyzed to describe PrEP cascade, follow-up, and PrEP trajectories. A multivariable logistic regression identified factors associated with PrEP early adopters.

RESULTS: Although 98% of eligible FSWs were interested in PrEP, only 62% initiated PrEP, and 39% renewed it at least once.
Follow-up was very short (<6 months) for 48%, short (6-12 months) for 15%, seasonal (>12 months with a gap of 6 months between two visits) for 31%, and regular (>12 months with no gap) for 6%.
PrEP initiation was high among FSWs with regular (96%), seasonal (83%), or short (80%) follow-up, vs. 39% for those with very short follow-up. Among those who initiated, only 88% (regular), 70% (seasonal), 74% (short), and 35% (very short) renewed PrEP.
Among 148 FSWs with regular/seasonal follow-up, four PrEP trajectories were identified (cf. figure showing individual trajectories): 15% “never initiated” PrEP, 39% “initiated and were later not interested anymore”, 18% “re-initiated after non-interest”, and 28% “initiated and never expressed non-interest”, the last two groups being considered as “early adopters”. Factors associated with early PrEP adoption included the usual price of intercourse with clients <1500 FCFA (aOR=2.4 [1.2-4.8]) and working in brothels (aOR=3.2 [1.6-6.6]).

CONCLUSIONS: In PRINCESSE, loss-to-follow-up and seasonality of risks limited PrEP adoption. FSWs who were more precarious and less mobile became early adopters of this new prevention tool and could serve as potential ambassadors for promoting its uptake.

Résumé

Le cancer du sein est le cancer le plus fréquent au monde. Dans de nombreux pays des Suds, les ressources thérapeutiques sont faibles, ce qui amène certaines femmes en quête de soins à quitter leur pays. Avec une approche biographique mixte (à la fois qualitative et quantitative), nous retraçons les parcours de vie de ces femmes et cherchons à préciser ce qu'impliquent ces mobilités thérapeutiques en termes de vécus et de ruptures biographiques. Afin d'avoir une vision globale de ces circulations, et afin de saisir les raisons qui amènent certaines femmes à partir, nous menons cette recherche en France (région Ile de France) mais aussi en Afrique de l'Ouest (Mali, Bénin) et Asie du Sud-Est (Cambodge). Avec le souci constant de co-contruire nos connaissances, et en impliquant des chercheures, des soignantes et des associations de femmes, cette recherche souhaite contribuer à la lutte contre les inégalités sociales de santé et à la mise en lumière de ces cancers en Afrique de l'Ouest et en Asie du Sud-Est.

Abstract

HIV self-testing (HIVST) empowers individuals to decide when and where to test and with whom to share their results. From 2019 to 2022, the ATLAS program distributed ∼ 400 000 HIVST kits in Côte d'Ivoire, Mali, and Senegal. It prioritised key populations, including female sex workers and men who have sex with men, and encouraged secondary distribution of HIVST to their partners, peers and clients.

To preserve the confidential nature of HIVST, use of kits and their results were not systematically tracked. Instead, an anonymous phone survey was carried out in two phases during 2021 to estimate HIVST positivity rates (phase 1) and linkage to confirmatory testing (phase 2). Initially, participants were recruited via leaflets from March to June and completed a sociobehavioural questionnaire. In the second phase (September-October), participants who had reported two lines or who reported a reactive result were recontacted to complete another questionnaire. Of the 2 615 initial participants, 89.7% reported a consistent response between the number of lines on the HIVST and their interpretation of the result (i.e., ‘non-reactive' for 1 line, ‘reactive' for 2 lines).

Overall positivity rate based on self-interpreted HIVST results was 2.5% considering complete responses, and could have ranged from 2.4% to 9.1% depending on the interpretation of incomplete responses. Using the reported number of lines, this rate was estimated at 4.5% (ranging from 4.4% to 7.2%). Positivity rates were significantly lower only among respondents with higher education. No significant difference was observed by age, key population profile, country or history of HIV testing.

The second phase saw 78 out of 126 eligible participants complete the questionnaire. Of the 27 who reported a consistent reactive response in the first phase, 15 (56%, 95%CI: 36 to 74%) underwent confirmatory HIV testing, with 12 (80%) confirmed as HIV-positive, all of whom began antiretroviral treatment.

The confirmation rate of HIVST results was fast, with 53% doing so within a week and 91% within three months of self-testing. Two-thirds (65%) went to a general public facility, and one-third to a facility dedicated to key populations.

The ATLAS HIVST distribution strategy reached people living with HIV in West Africa. Linkage to confirmatory testing following a reactive HIVST remained relatively low in these first years of HIVST implementation. However, if confirmed HIV-positive, almost all initiated treatment. HIVST constitutes a relevant complementary tool to existing screening services.

Abstract

The history of the fight against AIDS has been marked by the development of major scientific and biomedical innovations: antiretroviral treatments, rapid screening tests, pre-exposure prophylaxis... However, these innovations have often been insufficient on their own to curve the epidemics. It has also been essential to innovate in terms of funding, organisation of services and care, advanced “out-of-hospital” strategies, human rights, taking account of the specific characteristics of the most vulnerable populations, etc.

Effective innovation will be ineffective if it is not adapted to the social, cultural, and legal constraints faced by populations and to the structural, organisational, and economic constraints of healthcare systems. Hence the need for a genuine science of implementation that is necessarily interdisciplinary and intersectoral.

Abstract

Background

Gay, bisexual, and other men who have sex with men (MSM) are disproportionately affected by HIV. In Africa, MSM face structural barriers to HIV prevention and treatment that increase their vulnerability to HIV acquisition and transmission, and undermine the HIV response. In this systematic review, we aimed to explore progress towards increases in HIV testing, improving engagement in the HIV treatment cascade, and HIV incidence reductions among MSM in Africa.

Methods

We searched Embase, MEDLINE, Global Health, Scopus, and Web of Science for cross-sectional and longitudinal studies reporting HIV testing, knowledge of status, care, antiretroviral therapy (ART) use, viral suppression, and HIV incidence among MSM in Africa published between Jan 1, 1980, and March 3, 2023. We pooled surveys using Bayesian generalised linear mixed-effects models, used meta-regression to assess time trends, and compared HIV incidence estimates among MSM with those of all men.

Findings

Of 9278 articles identified, we included 152 unique studies published in 2005–23. In 2020, we estimate that 73% (95% credible interval [CrI] 62–87) of MSM had ever tested for HIV. HIV testing in the past 12 months increased over time in central, western, eastern, and southern Africa (odds ratio per year [ORyear] 123, 95% CrI 101–151, n=46) and in 2020 an estimated 82% (70–91) had tested in the past 12 months, but only 51% (30–72) of MSM living with HIV knew their HIV status. Current ART use increased over time in central and western (ORyear 141, 108–193, n=9) and eastern and southern Africa (ORyear 137, 104–184, n=17). We estimated that, in 2020, 73% (47–88) of all MSM living with HIV in Africa were currently on ART. Nevertheless, we did not find strong evidence to suggest that viral suppression increased, with only 69% (38–89) of MSM living with HIV estimated to be virally suppressed in 2020. We found insufficient evidence of a decrease in HIV incidence over time (incidence ratio per year 096, 95% CrI 063–150, n=39), and HIV incidence remained high in 2020 (69 per 100 person-years, 95% CrI 31–276) and substantially higher (27–199 times higher) than among all men.

Interpretation

HIV incidence remains high, and might not be decreasing among MSM in Africa over time, despite some increases in HIV testing and ART use. Achieving the UNAIDS 95-95-95 targets for diagnosis, treatment, and viral suppression equitably for all requires renewed focus on this key population. Combination interventions for MSM are urgently required to reduce disparities in HIV incidence and tackle the social, structural, and behavioural factors that make MSM vulnerable to HIV acquisition.

Abstract

Background

Timely linkage to care and ART initiation is critical to decrease the risks of HIV-related morbidity, mortality and HIV transmission, but is often challenging. We report on the implementation and effectiveness of a linkage-to-care intervention in rural KwaZulu-Natal, South Africa.

Methods

In the ANRS 12249 TasP trial on Universal Testing and Treatment (UTT) implemented between 2012–2016, resident individuals ≥16 years were offered home-based HIV testing every six months. Those ascertained to be HIV-positive were referred to trial clinics. Starting May 2013, a linkage-to-care intervention was implemented in both trial arms, consisting of tracking through phone calls and/or home visits to “re-refer” people who had not linked to care to trial clinics within three months of the first home-based referral. Fidelity in implementing the planned intervention was described using Kaplan-Meier estimation to compute conditional probabilities of being tracked and of being re-referred by the linkage-to-care team. Effect of the intervention on time to linkage-to-care was analysed using a Cox regression model censored for death, migration, and end of data follow-up.

Results

Among the 2,837 individuals (73.7% female) included in the analysis, 904 (32%) were tracked at least once, and 573 of them (63.4%) were re-referred. Probabilities of being re-referred was 17% within six months of first referral and 31% within twelve months. Compared to individuals not re-referred by the intervention, linkage-to-care was significantly higher among those with at least one re-referral through phone call (adjusted hazard ratio [aHR] = 1.82; 95% confidence interval [95% CI] = 1.47–2.25), and among those with re-referral through both phone call and home visit (aHR = 3.94; 95% CI = 2.07–7.48).

Conclusions

Phone calls and home visits following HIV testing were challenging to implement, but appeared effective in improving linkage-to-care amongst those receiving the intervention. Such patient-centred strategies should be part of UTT programs to achieve the UNAIDS 95-95-95 targets.

À deux jours de la Journée mondiale du SIDA du 1^er décembre, RFI donne la parole à des chercheurs et des acteurs de terrain, mobilisés pour lutter contre la maladie. Le VIH touche environ 38,4 millions de personnes dans le monde, dont plus des deux tiers (25,6 millions) dans la région africaine.

Au travers de programmes qui intègrent les patients et les associations, nous verrons comment cette dimension communautaire constitue l'un des piliers des projets conduits à l'international, par l'agence ANRS/Maladies Infectieuses Émergentes, qui coordonne et finance la recherche française sur ces pathologies. L'ANRS avait été, à son origine, en 1988, créée face à l'urgence de l'épidémie à VIH.
