Résumé

La génération actuelle de jeunes (18-30 ans) arrive à un âge où la façon dont la sexualité est perçue et vécue change profondément. Des recherches ont montré que les jeunes connaissent des changements d'orientation sexuelle au cours du temps (aussi appelés fluidité sexuelle), notamment des changements d'attirances, d'identités et/ou de comportements sexuels. De nouvelles données démontrent aussi que la fluidité sexuelle est un déterminant essentiel des inégalités en matière de santé sexuelle, de santé mentale, de consommation de substances et de maladies chroniques. De plus, des études suggèrent que des facteurs contextuels (par exemple, les normes de genre, le climat politique) peuvent influencer les expériences de fluidité sexuelle, mettant en avant le besoin de mener des études comparatives entre pays pour mieux appréhender le rôle de ces facteurs. Des approches de recherche innovantes sont ainsi nécessaires pour comprendre comment évolue la sexualité des jeunes dans le contexte actuel.

Pour combler ces lacunes, notre objectif est de développer une nouvelle étude de cohorte longitudinale sur la fluidité sexuelle chez les jeunes au Canada, en France et au Royaume-Uni (RU). Pour cela, une étude pilote permettra d'identifier les mesures et les méthodes adaptées pour évaluer au mieux la diversité et la complexité des modèles et des trajectoires de fluidité sexuelle chez les jeunes. Cette étude offrira la possibilité d'établir des partenariats avec des leaders communautaires et des chercheurs locaux et internationaux qui faciliteront le développement de notre futur programme de recherche sur la fluidité sexuelle.

Abstract

Background

Pre-exposure prophylaxis (PrEP) is recommended by the WHO for HIV prevention among female sex workers (FSWs). A study conducted in 2016–2017 in Côte d'Ivoire showed that if PrEP is acceptable, FSWs also have many uncovered sexual health needs. Based on this evidence, the ANRS 12381 PRINCESSE project was developed in collaboration with a community-based organization. The main objective is to develop, document, and analyze a comprehensive sexual and reproductive healthcare package among FSWs in Côte d'Ivoire.

Methods

PRINCESSE is an open, single-arm interventional cohort of 500 FSWs in San Pedro (Côte d'Ivoire) and its surroundings. Recruitment started on November 26^th, 2019 and is ongoing; the cohort is planned to last at least 30 months. The healthcare package (including HIV, hepatitis B, and sexually transmitted infection management, pregnancy screening, and contraception) is available both at mobile clinics organized for a quarterly follow-up (10 intervention sites, each site being visited every two weeks) and at a fixed clinic.

Four waves of data collection were implemented: (i) clinical and safety data; (ii) socio-behavioral questionnaires; (iii) biological data; and (iv) in-depth interviews with female participants. Four additional waves of data collection are scheduled outside the cohort itself: (i) the medical and activity records of Aprosam for the PRINCESSE participants; (ii) the medical records of HIV+ FSW patients not participating in the PRINCESSE cohort, and routinely examined by Aprosam; (iii) in-depth interviews with key informants in the FSW community; and (iv) in-depth interviews with PRINCESSE follow-up actors.

Discussion

The PRINCESSE project is one of the first interventions offering HIV oral PrEP as part of a more global sexual healthcare package targeting both HIV- and HIV+ women. Second, STIs and viral hepatitis B care were offered to all participants, regardless of their willingness to use PrEP. Another innovation is the implementation of mobile clinics for chronic/quarterly care. In terms of research, PRINCESSE is a comprehensive, interdisciplinary project combining clinical, biological, epidemiological, and social specific objectives and outcomes to document the operational challenges of a multidisease program in real-life conditions.

Trial registration

The PRINCESSE project was registered on the Clinicaltrial.gov website (NCT03985085) on June 13, 2019.

Résumé

Les travailleuses du sexe (TS) constituent une des populations prioritaires des programmes de lutte contre le VIH. Pendant longtemps, le focus a été la promotion du préservatif et, dans les années 2000, le dépistage et le traitement des TS infectées. Ces politiques ont-elles véritablement enrayé la vulnérabilité de ces femmes ? Cette communication présentera des résultats issus d'entretiens menés en 2016 (projet ANRS 12361 PrEP-CI) sur différents sites prostitutionnels d'Abidjan et de San Pedro. Les TS sont en réalité exposées au VIH dans de nombreuses situations, notamment avec leurs partenaires réguliers, avec des clients les agressant ou payant davantage d'argent pour un rapport non protégé. Leur faible pouvoir de négociation s'inscrit dans un contexte de domination masculine. À ces vulnérabilités de genre s'ajoutent les vulnérabilités matérielles liées à leurs conditions de travail. Leur mobilité importante pour contourner leur marginalisation sociale est également un facteur de fragilisation. Les nouveaux outils de prévention tels que la prophylaxie préexposition pourraient selon certains compenser ces vulnérabilités et contribuer à leur émancipation en termes de santé. Si cet outil pourrait être approprié, il convient cependant de ne pas considérer ces femmes uniquement au prisme du VIH, puisqu'elles ont de nombreux autres besoins en santé sexuelle et reproductive et font face plus globalement à un problème d'accès aux droits humains fondamentaux.

Abstract

Aim

In West Africa, most countries have mixed HIV epidemics; new tools such as pre-exposure prophylaxis (PrEP) should target in priority most affected populations, particularly female sex workers (FSWs). This paper describes sexual and reproductive health (SRH) needs of FSWs in Côte d'Ivoire to inform the future implementation of PrEP in this population.

Method/Issue

The ANRS 12361 PrEP-CI cross-sectional and mixed-methods study was designed and implemented with two Ivorian community-based organizations. 1000 FSWs completed a standardized questionnaire assessing women's sociodemographic characteristics, sexual practices and behaviors, use of community health services, a priori acceptability of PrEP. Twenty-two in-depth interviews and eight focus group discussions were conducted at prostitution sites. FSWs were interviewed about risky practices and sexual behaviors, experiences with violence and discrimination, attitudes regarding HIV and sexually transmitted infections (STIs), and barriers to SRH services.

Results/Comments

The quantitative and qualitative results showed that FSWs were highly exposed to HIV despite their use of condoms with clients. 91% did not use condoms with their regular partner, despite their acknowledged concurrent sexual partnerships. 23% accepted condomless sexual intercourse for a large sum of money, especially when they had had few previous clients. Moreover, FSWs faced many unmet needs regarding SRH beyond HIV prevention and treatment. Inconsistent condom use exposed FSWs to STIs and undesired pregnancies. However, the prevalence of contraceptive use was low (40%) due to fear of contraception causing sterility. FSWs faced obstacles in accessing SRH care and preferred advice from their peers or self-medication.

Discussion

Implementing PrEP among FSWs in West Africa, such as in Côte d'Ivoire, constitutes an opportunity to consider the chronic follow-up of HIV-negative FSWs. PrEP initiation should not condition access to SRH services; conversely, SRH services could be a way to attract FSWs into practicing HIV prevention. Our results highlight the importance of developing a people-focused approach that integrates all SRH needs when transitioning from PrEP efficacy trials to implementation.

Female sex workers and HIV acquisition in Côte d'Ivoire: the burden of precariousness and working conditions (ANRS 12361 PREP-CI)

Abstract

Background

Female sex workers (FSW) are a population at high risk of HIV acquisition. New tools to diagnose recent HIV infection may be used to better describe risk of HIV acquisition in this group. The ANRS 12361 PrEP-CI project aimed to estimate HIV incidence and describe risk factors among FSW in Côte d'Ivoire.

Methods

The study was conducted between September 2016 and March 2017 among FSW aged ≥18 years old in Abidjan and San Pedro. Two rapid tests for recent HIV infection (Determine®, Alere and Vikia®, bio Mérieux) were offered to FSW; in case of positive result, a dried blood sport was collected and window period of infection determined using a test for recent infection (EIA-RI) adapted to the Ivorian context. A standardized sociodemographic questionnaire was then administered to FSW by female peer educators, completed with qualitative interviews and focus groups with 60 FSW.

Results

1000 SW (400 in San Pedro and 600 in Abidjan) with a median age of 25 years (interquartile range: 21-29) were included in the study and screened for HIV. Of these, 39 were diagnosed with HIV, 7 of which were newly infected (mean duration of infection was 113 days), corresponding to an HIV incidence of 2.3% [95% confidence interval: 1.6 - 3.1%]. The incidence was higher in San Pedro (3.3% [2.2-4.5]) than in Abidjan (1.6% [1.1-2.2]); this can be explained by qualitative data in which FSW in San Pedro expressed difficulties (distance, time, costs ...) in accessing health care services and prevention methods, particularly regarding condoms. HIV infection was also associated with vulnerability: the incidence was higher among those who charged ≤ 2000 CFA francs per intercourse (3.3% vs. 0.7%), those who performed on the streets or in hotels (5.4% and 4.2%, respectively, vs. 3.9% in brothel and 0.8% in bar – drinking spot), and among those who had more than 5 clients the last working day (6.1% vs. 1.8% among those who had under 5 clients) and who could be more difficult to identify and refer to HIV prevention services by female peer educators. Finally, FSW under <25 years old were at higher risk of infection (2.8%) than older ones (1.8%), and HIV incidence was associated with other sexually transmitted infections (STI) (2.5% in those who had ≥ 1 STI in the last 12 months vs. 1.9%). Contrary to expectations, an extensive sex work experience did not reduce exposure to HIV, when we compared FSW who had been working for more than 3 years with the ones who had newly arrived in the sex work industry (2.5% vs. 2.0%).

Conclusion

This study is one of the first to estimate HIV incidence in Côte d'Ivoire among FSW. This key population is much more exposed than the rest of the general population (2.3% vs. 0.06% estimated by UNAIDS). New infections seem to concentrated among younger FSW in precarious situations and working in remote areas, such as San Pedro area. These vulnerable subgroups should therefore be a priority target for prevention programs such as pre-exposure prophylaxis (PrEP) as recommended by WHO.

Abstract

In the past few years, two biomedical interventions have been added to classical tools of HIV prevention: early treatment (TasP) and oral pre-exposure prophylaxis (PrEP). Despite the high quality evidence from recent PrEP trials, research on how best to deliver PrEP in ‘real life' in different populations and settings remains crucial.
In Côte d'Ivoire, female sex workers (FSWs) have very high HIV prevalence (29%, UNAIDS 2015) and do not systematically use condoms: a 2014 survey showed condom use of 90% at last sexual act with a client and 15% with ‘non-client' partner (Bamba et al. 2014). This strongly suggests that PrEP could be of great value to protect the ‘remaining risky sexual acts'.

In this context, we conducted a qualitative survey among 60 FSWs reached by two community NGOs, to identify potential obstacles to regular clinical monitoring of HIV negative FSWs that any incoming program of PrEP will have to take into account. Results showed that barriers remain for HIV negative FSWs to access services offered in dedicated health centres. Implementing a program of PrEP for this population necessitate rethinking health care services and delivery system, as PrEP cannot be delivered without implementing a global package of sexual health services.

MIGMAC - Mesurer les Inégalités de Genre des Ménages ouest-Africains en matière de Consommation

Ce projet s'inscrit dans le cadre d'une collaboration entre AFRISAT, le CEPED (Centre Population et Développement UMR 196 Université Paris Descartes INED IRD) et l'IRD.

Contexte

Le concept de « féminisation de la pauvreté » a été mis en exergue au moment de la conférence de Pékin sur les femmes en 1995. Il s'appuie sur le fait que les ménages dirigés par une femme seraient plus pauvres que les autres, constat non vérifié dans plusieurs pays dont la Mauritanie ou le Sénégal, entre autres parce que le statut de chef de ménage recouvre des situations diverses. La question de la féminisation de la pauvreté est cependant à l'origine de recherches micro-économiques particulièrement fécondes sur les inégalités à l'intérieur des ménages.

L'importance de la dimension genre dans le développement a été réaffirmée dans le cadre des Objectifs du Millénaire pour le Développement (OMD) au travers de l'objectif 3 qui vise à promouvoir l'égalité des sexes et l'autonomisation des femmes.

Objectif

Ce projet vise à analyser les relations entre la structure démographique des ménages, en particulier le nombre d'actifs « contributeurs » de chaque sexe, et la structure des dépenses de consommation des ménages. Existe-il un effet de genre sur la consommation des ménages ?
Il s'agit d'une analyse secondaire des données des enquêtes sur les dépenses des ménages (EDM) réalisées dans le cadre de la rénovation de l'IHPC (indice harmonisé des prix à la consommation), enquêtes menées dans les capitales des huit pays de la zone UEMOA (union économique et monétaire ouest-africaine : Bénin, Burkina Faso, Côte d'Ivoire, Guinée-Bissau, Mali, Niger, Sénégal et Togo).

Les analyses seront d'abord menées sur les données de 2008 au travers d'un comparatif régional. Dans un second temps, l'analyse pourra être élargie pour prendre en compte les évolutions du phénomène en intégrant les données des enquêtes précédentes (EDM 1996 et PARSTAT 2001).
